How to Order Birth Certificates
Clermont County Public Health can issue certified copies of all birth certificates filed in the State of Ohio (not just for Clermont County residents). All birth certificate orders are NON-REFUNDABLE AND CANNOT BE EXCHANGED. To obtain a certified copy of an Ohio birth certificate, you have several options:
Order Online
For Babies Born Outside a Birthing Facility
Whether you are planning a home birth or you unexpectedly had your baby outside the hospital, there are some steps that you will need to take to file your child’s birth certificate. For children born at the hospital, this process is typically initiated and completed by hospital staff. However, when your child is born at a location other than a hospital, it is the mother’s responsibility to initiate and complete the required birth certificate information.
Upon the birth of a child, the State of Ohio allows one year for the birth certificate information to be filed with the local health district. Ideally, you will want to file your child’s birth certificate within 10 days of birth to avoid additional paperwork.
Please follow the steps below to file the certificate:
- Download and complete the Newborn Birth Certificate Packet.
- Contact Clermont County Public Health’s Vital Statistics Registrar at (513) 732-7499 to schedule an appointment to file a home birth certificate.
How to Order Death Certificates
Did you know you can order death certificates directly from the health department? Clermont County Public Health can issue certified copies of all death certificates files in the State of Ohio from 1971 to present. Please call our office at (513) 732-7499 ahead of your visit to ensure that we can print a death certificate that was created in 2006 or earlier.
Death certificates cannot be ordered online. To obtain a certified copy of a death certificate, please complete the Birth/Death Certificate Request Form. This form also is available in our lobby. Typically, a request for a vital record can be fulfilled in our office in less than 15 minutes. Should you require the certificate to be mailed to you, please include a self-addressed stamped envelope so that we may mail the certificate back to you. If an envelope with correct postage is not included with your request, your request cannot be fulfilled.
If the Social Security Number is required to be listed on the death certificate, please include a copy of your ID and any additional documentation. Please see the application for details about the social security number.
